Carotid Endarterectomy in Bilateral Carotid Stenosis



Abstract
The authors experienced bilateral carotid endarterectomy in one patient. The patient was 69-year-old woman who had several transient ischemic attacks (TIA's) and anterior chest pain. She had been suffered from hypertension and hyperlipidemia
since
five
years ago. Preoperative carotid angiogram showed severe narrowing (90 percent) of the left proximal internal carotid artery. Under the intraoperative monitoring of electroencephalograms. we performed left carotid endarterectomy first due to the
left
hemispheric Tia's. Six months laten right carotid endarterectomy was done because the degree of right carotid stenosis increased from 70 percent to 90 percent and new infarcts on right periventricular and subcortical white matters developed. She
had
coronary artery disease. So coronary artery bypass graft was performed. The patient recovered without postoperative morbidity. We reviewed about the indications and surgical techniques of carotid endarterectomy.
